10799 Pharmacy Smoking Cessation (Berkshire Area) Thames Valley LPC Ongoing 01/10/2012

The Stop Smoking service in Berkshire was awarded to Solutions4Health (S4H)in January 2012. S4H began providing the service from October 2012.

Pharmacy contractors have several options on what level they engage with the stop smoking service. They can:

  • sign up to provide the full service in their pharmacy;
  • sign up to allow S4H to provide the service in their pharmacy; or
  • sign up to refer patients to S4H

Pharmacies that offer the full service in the pharmacy provide one to one support and advice to people who want to give up smoking on a weekly basis for six to eight weeks. NRT and/or varenicline may be used to achieve 4 and 12 week quitters. Smoking status should be validated using a CO test, and at least 85% of quitters must be proved to be verified this way.

With all of the above options pharmacists can also sign up to provide varenicline to patients referred to them by Solutions4Health under a PGD.

Stop Smoking Advisers must attend an appropriate S4H Adviser course (2 day course) that adheres to training standards approved by NICE or the Health Development Agency. Those working as an adviser will be required to provide evidence of their training certificates.

Where a Stop Smoking Advisor has not provided any stop smoking support for 12 months, the training course must be repeated and completed.

Stop Smoking Advisers should have completed the NCSTC Stage 1 online training and completed Level 1 and Level 2 S4H training (classroom led). It is also recommended that Advisers complete the Motivational Interviewing training and the more in-depth NRT training; however, these are optional. S4H will offer regular training and refresher courses throughout the year to develop the skills of Advisors including Pharmacotherapy.

This service is only available for patients who either live or work in the following areas: West Berkshire, Wokingham, Reading, Windsor, Ascot, Maidenhead, Bracknell and Slough

The service aims to provide a quality, evidence based intermediate stop smoking treatment for smokers in East Lancashire. It will improve access to support and increase patient choice, particularly in areas of high deprivation, and contribute to reducing health inequalities.

The integrated service provides one to one cessation treatment for smokers by appropriately trained staff, and will link into other lifestyle health improvement services as part of a wider programme to improve the health of the local population.

Carbon monoxide monitors are supplied, but service providers are responsible for arranging calibration of the monitors in line with manufacturers instructions.

The service offers support for at least 12 weeks dependent on client needs. Repeat quitters are welcome to re-attend the service after a smoking advisor has assessed their readiness to make another quit attempt.

The service is capped at 50 quits per annum, any number over this will be reviewed.

Four levels of service are available to all pharmacies in Bolton:

  • Level 1 Service – Stop Smoking Voucher Scheme

Clients are issued with a voucher from Bolton Stop Smoking Service, which they take to a participating pharmacy to be issued with the nicotine replacement therapy (NRT) that is written on the voucher (NRT type can however be changed if requested by client).

  • Level 2 Service – NRT Support and Supply

Clients are supported with behavioural support and with appropriate NRT pharmacotherapy to set a quit date and their success is measured at four weeks and 12 weeks through CO verification.

  • Level 3 Service – Varenicline (Champix) – Supply Only via PGD

Clients are issued with a voucher from Bolton Stop Smoking Service, which they take to a participating pharmacy to be issued with varenicline (Champix). Pharmacists must have signed the PGD in order to provide this medication.

  • Level 4 Service – Varenicline (Champix) – Support and Supply via PGD

Clients are identified as wanting to stop smoking and are supported with behavioural support and supply of varenicline (Champix), to set a quit date and their success is measured at four and twelve weeks through CO verification. Pharmacists must have signed the PGD in order to provide this medication.

The service to be provided is a Level 2 Stop Smoking Service which involves multi-session interventions with a total potential client contact time being a minimum of 1.5 hours from pre quit preparation to 4-weeks post quit. This will involve offering weekly support to individuals committing to quit smoking from their quit date until they have been quit for 4-weeks or until they relapse, if this is less than 4-weeks after the quit date.

The service will consist of up to seven consultations with the Practitioner. Each client will receive a course of treatment tailored to his or her individual requirements.

The local Stop Smoking Service also has an agreement with local community pharmacies for the dispensing of NRT products through a voucher scheme (as opposed to FP10 prescribing). The voucher scheme allows Stop Smoking advisors to prescribe NRT products through the use of a voucher and for local community pharmacies to dispense the voucher and claim a payment (minus any prescription fees collected).

Primary Care staff providing stop smoking services must be trained to NHS Centre for Smoking Cessation Training (NCSCT) Standards and have been assessed as competent to deliver Level 2 (one-to-one) stop smoking support. Accrediated Practitioners will have their names on an enhanced service provider list kept by the Stop Smoking Service , on whose behalf they are providing the service.

Accreditation is gained by completing the NCSCT Stage 1 online assessment, atttending a 1/2- day Stop Smoking Practitioner course provided by the Stop Smoking Service and finally the Stage 2 online assessment. No fee will be charged for attending the accreditaion training There will not be a charge for update training either.

Practitioners must attend one accreditation update session per year.

Pharmacies Professionals ONLY – Self Accreditation is available as an option to attending the annual update. The self accredetation will be completed by the pharmaacy professional in the form of a CPPE self declaration of competence. The pharmacy professional must satisfy the requirements of the self-declaration of competence for Smoking Cessation Intervention Service and print and sign the personal Declaration of Qualification and Competence to deliver the Stop Smoking Service.

If a Practitioner has not provided any stop smoking support for one year, Level 2 training must be repeated.

Community Pharmacy teams will deliver brief interventions, assessments and support to clients who are motivated and want to stop smoking. The following levels of support are available to pharmacy clients wishing to quit smoking:

  • Brief intervention - usually the first contact for a customer with a member of the pharmacy team, who will spend between 1-3 minutes raising the issue of smoking and for a motivated quitter refer them to either pharmacy based smoking cessation support or a specialist smokefree service for support.
  • Pharmacy based support - offered by a minimum level 2 trained smokefree adviser (can be anyone employed by the pharmacy who has received training to the required standards for less dependent and motivated quitters). A pharmacy may supply NRT products to customers who have been provided with a letter of referral issued by a Bucks Smokefree Support Service (BSSS) specialist advisor.

Anyone wishing to become a Stop Smoking Service Advisor is required to complete the accredited training endorsed by the Commissioners and provided by the Stop Smoking Service (as set out by the NHS Centre for Smoking Cessation and Training). In addition, all advisors are required to meet update requirements, currently attendance at annual update.

In the circumstance that an advisor has been inactive for a period of one year or more they will be required to undertake refresher training.

Staff involved in delivery of ‘rolling groups’ or ‘drop in clinics’ must be trained to national standards and such groups must be delivered by advisors with sufficient expertise to support quitters at different stages of the quitting process simultaneously. Training to provide this support is in addition to level 1 and level 2 accredited stop smoking advisor training.

Deliverers of the service can be pharmacists or other accredited members of pharmacy staff. Pharmacists or staff will need to attend two days of training provided by Coventry Stop Smoking (Healthy Lifestyles) service of Coventry & Warwickshire Partnership Trust. Since 2010, the NCSCT piloted their training scheme which is made up of online modules and face to face skills training. All subcontractors will be required to undertake and pass Level 1 and Level 2 assessments in order to deliver stop smoking services. Coventry & Warwickshire Partnership Trust Stop Smoking (Healthy Lifestyles) service will support and assist those deliverers within the pharmacy to complete and achieve this accredited training.

Deliverers of the service must have a valid Disclosure and Barring Service (DBS) check with their place of work.

One to one support is provided to clients who wish to give up smoking. An initial consultation is held, which will normally last around 30 minutes. If considered appropriate for nicotine replacement therapy (NRT), the client will be supplied with NRT and counselled on its use. Follow up consultations (these can take place on the telephone) should be made and a further supply of NRT made if appropriate. Anyone who attends such a meeting and passes the training assessment can provide the Stop Smoking service as an intermediate level advisor. In addition, all pharmacists and registered technicians providing the service must have read:

  • "Helping smokers to stop: advice for pharmacists in England";
  • "Brief interventions and referral for smoking cessation in primary care and other settings" (Public Health Intervention Guidance No 1); and
  • any other relevant national or local guidelines, as and when notified.

In addition, all staff providing the service must complete the NHS Centre for Smoking Cessation and Training (NCSCT) Stafe 1 Training and Assessment programme (knowledge).

The main aim of this service is to support the reduction of smoking prevalence in the geographical area covered by Devon County Council. In addition, it enables smokers to access a choice of high quality support to stop smoking to best suit their needs.

The pharmacy contractor will provide one or more in-house Stop Smoking Advisers, trained and registered with the Devon Level 3 Stop Smoking Specialist service. The contractor will offer clients stop smoking appointments with a Level 2 (Intermediate) stop smoking adviser within their own premises. 

The contractor will also dispense nicotine replacement therapy (NRT) for other local NHS stop smoking services in the community (eg dental advisers, school nurses) where a Devon Stop Smoking Service Letter of Recommendation has been presented.

NRT is recommended to be dispensed for 2 weeks, 2 weeks, 4 week and an optional further 4 weeks if required. NRT may be dispensed under this service for a maximum of 12 weeks supply. 

The initial consultation should include:

  • a carbon monoxide (CO) test and an explanation of its use as a motivational aid;
  • an explanation of the benefits of quitting smoking;
  • a description of the main features of tobacco withdrawal and the common barriers to quitting;
  • treatment options that are proven effective;
  • a description of the support offered – i.e. its aims, length and benefits; and
  • an agreement on the chosen treatment pathway.

Follow up consultations should include appropriate support strategies to help the person quit, further supplies of medication where appropriate and CO monitoring (the four-week follow up should include self-reported smoking status, followed by a CO test for validation).

18533 Stop Smoking Interventions in Pharmacy (Plymouth Area) Devon LPC Ongoing

This service is part of a pilot project that will run in parallel with the Healthy Living Pharmacy (HLP) scheme in Plymouth. The nature of this pilot means that only eligible pharmacies can operate under this service. The intention is for this service to be rolled out with full access to all pharmacies based in Plymouth once this pilot has been reviewed.

The practice will arrange an initial appointment for each patient wishing to use the community pharmacy’s stop smoking service to set a quit day (20-30 minute appointment). Nicotine replacement therapy (NRT) can be recommended and issued by the pharmacy. Varenicline can be recommended by the pharmacy and a signed letter of recommendation for their GP should be given to the patient where suitability for the medication has been checked by an appropriate pharmacist.

The community pharmacy will arrange two follow-up appointments over the 8 week treatment programme in line with prescribing recommendations (10-20 minute appointments). The four week appointment is the most important because this is where the outcome is determined.

The Stop Smoking Practitioner will CO validate all quit attempts using a smokerlyser, which will be provided free of charge to the Community Pharmacy by the Plymouth NHS Stop Smoking Service .

Unsuccessful quitters may be seen again under this service but repeating/re-starting the service should be in line with local recommendations.

18537 Stop Smoking Service (Torbay Area) Devon LPC Ongoing

Nicotine replacement therapy (NRT) may be supplied under this scheme for a minimum of 6 weeks and a maximum of 12 weeks. It is recommended that this is dispensed for 2 weeks, 2 weeks, 4 weeks and 4 weeks.

Initial appointment - At the initial appointment the client should be provided with an overview of the service and their CO level should be monitored using a Smokerlyser. Different therapy options should be discussed and NRT dispensed if appropriate. A quit date should be agreed.

2nd appointment (approx 7 days after quit date) - Progress should be reviewed and CO levels monitored. Checks should be made to ensure the client is not suffering with side effects from the NRT and coping strategies for cravings should be discussed.

3rd appointment (approx 14 days after quit date) - Same as appointment 2 plus a further 2 weeks supply of NRT should be made.

4th appointment (approx 21 days after quit date) - If the client has made it this far, they can be recorded as a successful quitter. NRT should be provided in lower dose and four weeks worth of NRT should be supplied. CO level should be monitored.

Further appointments - The client may continue to visit the pharmacy for NRT under the contract, up to a total of 12 weeks’ supply.

Follow up training must be attended annually by at least one member of the pharmacy team who will if necessary cascade the messages to other members of the team who are involved in the service. Patients who wish to use bupropion or varenicline should be enrolled for support only, their medication will be provided by their GP subject to medical suitability.

Following the initial assessment weekly client contact should be provided, preferably face to face, (telephone contact is acceptable if face to face is not possible). Each contact will offer on-going behavioural support, the monitoring of carbon monoxide levels and the discussion of treatment effectiveness.  

A face to face consultation should be standard practice at the 4-week review which must take place between 25 to 42 days after the quit date to CO verify patients’ quit status.  Clients whose follow-up dates fall outside this time range may not be counted.  Arrangements for the remaining course of treatment should also be made, but are dependant on the individual remaining quit.

The Level 1 service comprises Brief Intervention or (opportunistic advice) which can be delivered in a confidential space when the opportunity presents itself. The Specialist Stop Smoking Service provides training for Level 1 advisors. The Level 1 service consists of:

  • offering brief, or very brief, advice to all smokers; and
  • referring those who are ready to quit to a Level 2 Stop Smoking Advisor – which may be in the pharmacy or another pharmacy.

Brief intervention is considered to be part of normal care. It is not funded separately.

Level 2
The Level 2 (intermediate) service comprises the provision of behavioural support by trained and registered Stop Smoking Advisors together with pharmacotherapy where indicated.

Whenever possible and where appropriate, pharmacy service providers should facilitate onward referral to specialist treatment services.

Pharmacists or other appropriately trained staff should provide direct input wherever possible to promote their smoking cessation service.

Advice and support must take place in a private or quiet area of the pharmacy. There will be a requirement to share relevant information with all professionals involved in the treatment of individuals – this within the bounds of pharmacists professional confidentiality guidelines.

Training in smoking cessation skills and techniques is provided by the CWP West Stop Smoking Service for Pharmacists (and those who would have achieved accreditation by the 1-day training date) and pharmacy technicians/dispensers/healthcare assistants (HCAs) trained to a minimum of NVQ level 2 or are currently training for it.

The level of training provided by the CWP West Stop Smoking Service differs slightly dependant on whether it is for a Pharmacist or Technician/Dispenser.

Pharmacists training requirements
The training for Pharmacists consists of a 1-day face-to-face programme run by the CWP West Stop Smoking Service and an open learning programme provided by the Centre for Pharmacy Postgraduate Education (CPPE) which is the underpinning of all on-going Pharmacists training. The internet–based CPPE National Centre for Smoking Cessation and Training (NCSCT) Module for Smoking Cessation provides Pharmacists with the necessary underpinning knowledge to provide a quality enhanced stop smoking service and should be completed before attending the 1-day face-to-face training.

Pharmacists should also read:

  • “Helping smokers to stop: advice for Pharmacists in England”
  • “Brief Interventions and Referral for Smoking Cessation in Primary Care and other Settings” (Public Health Intervention Guidance No 1).

Technicians / Dispensers / HCA’s Training Requirements
Technicians and Dispensers are also required to complete the 1-day face-to-face programme run by the CWP West Stop Smoking Service. Before attending the 1-day training they must also complete the internet–based NCSCT Module for Smoking Cessation via the NCSCT’s website on www.ncsct.co.uk. This training is currently free of charge. All training covers the standards as outlined by the NCSCT.

In order to achieve the specific competences, Technicians, Dispensers and Healthcare Assistants will also be required to supplement their learning through additional reading provided on the course and time shadowing core specialist stop smoking advisors during their interventions with clients (if permitted by the pharmacist / pharmacy manager).

The objective of the service is to provide free smoking cessation support to housebound patients with chronic obstructive pulmonary disease (COPD) and other long term conditions.

Service outline:

  • Stage 1 Initial assessment*
  • Stage 2 Quit Day*(set by the patient)
  • Stage 3 Support following Quit Day
    (usually by phone, but can be a visit as part of the DSC specification)
  • Stage 4 Four week Quit follow up appointment i.e. 4 weeks after Quit Day*
  • Supply 4 weeks of NRT and inform the patient to phone the pharmacy to arrange an additional 4 week supply of NRT if required.
  • Stage 5 Supply and deliver to the patient the remaining 4 weeks of NRT.

*A face to face consultation is required.

This is a time limited pilot project, funded by GMHSCP (Greater Manchester Health and Social Care Partnership). The aim of ‘Swap to Stop’ is to offer current smokers who live in deprived areas of Salford to switch to an e-cigarette, free of charge.

Pharmacies taking part in the pilot will be:

  • redeeming voucher postcards from current smokers, and encouraging them to switch to an e-cigarette;
  • conducting a carbon monoxide test to interested participants to confirm they are a current smoker, and at regular intervals throughout the pilot project;
  • being a distribution point for participants to redeem their free e-cigartette 'starter kit' (chosen e-cig, 2 weeks’ supply of vape liquid from a range of flavours and strength), providing advice and guidance around choice of e-cigarette and vape liquid and strength; and

The new Camden and Islington Stop Smoking Service offers people who would like to stop smoking the opportunity to access various levels of support suited to their lifestyle and individual preferences. The service is 'e-cigarette friendly' and offers support to people wishing to stop smoking with the help of self-purchased e-cigarettes.

Providers may offer a tier 2 service to any eligible smoker, that is any smoker motivated to quit who is aged 13 years or over and lives, works or studies in Camden or Islington, and/or is registered with a Camden or Islington GP. The tier 2 service consists of evidence-based stop smoking behavioural support and pharmacotherapy (NRT), with the client’s smoking status recorded at 4 weeks (25 – 42 days) after their set quit date. A tier 2 stop smoking intervention consists of 6 one-to-one sessions of behavioural support and advice/ supply of pharmacotherapy, with a minimum of 2 sessions delivered face-to-face (quit date and outcome date).

The service will provide an integrated, safe, effective, person-centred and evidence-based stop smoking service, which is of the highest possible quality to ensure it contributes to improving the health of our population and tackles health inequalities.

Primary care is a key setting for stop smoking interventions. The public increasingly expects health professionals to ask about / question their smoking. It is intended that this model will recruit smokers opportunistically during routine pharmacy visits. Pharmacy teams are ideally placed to become actively involved in stop smoking support. There are opportunities to intervene with all visitors to the pharmacy, and providing both preventive advice and treatment.

A redesign of stop smoking services has recently been undertaken for stop smoking support across Blackpool. The agreed new model of stop smoking support for Blackpool will provide a light touch universal element and a proactive targeted approach to provide support for priority groups. A summary of the new proposed service model is detailed in the attached (Appendix A).

The Community Pharmacy Led Stop Smoking Service sits within level 2 of the model, offering a universal service to individuals who would like face-to-face stop smoking support which they can easily access within their local community.
