PharmOutcomes

PharmOutcomes is a web-based system which helps community pharmacies provide services more effectively and makes it easier for commissioners to audit and manage these services. By collating information on pharmacy services it allows local and national level analysis and reporting on the effectiveness of commissioned services, helping to improve the evidence base for community pharmacy services. 

The system is highly flexible, in that it allows the ‘commissioner’ to design services within PharmOutcomes which can then be made accessible to community pharmacies commissioned to provide that service.The system provides a whole system for provision, invoicing and management of any locally commissioned services. Services can be designed from scratch, or can be based on a suite of national templates or local templates which other commissioners have designed and have made available for use on the system.Currently the system provides templates for a range of services, which are listed below, but its innate flexibility allows other services to be easily designed within the system. All of these services can be edited after importing to meet specific local needs or they will simply work “out of the box” as they are.PharmOutcomes is provided by Pinnacle Health LLP in partnership with PSNC.

A number of other services created by local commissioners can be copied between commissioners on request.

The system allows pharmacy teams to make records of their locally commissioned services and payment claims are automatically created on a monthly basis. The commissioner can view live data on the services at any time, consequently assisting with service management.

The system is also available as Outcomes4Health, which provides access to an interface which is non-partisan and removes references to pharmacy but connects to the same database. This allows the use of the system by other healthcare professionals and third parties, with interest having been received from optometrists, commissioners of Health Check services and Third Sector providers.

Quality Payments Scheme support

Community pharmacy contractors are now able to access support on PharmOutcomes for the Quality Payments Scheme.

This support is available to all contractors as PSNC has agreed to use their licence to provide access to the Quality Payments Scheme framework and asthma referrals service on PharmOutcomes.

Quality Payments Scheme framework

A Quality Payments Scheme framework has been created on PharmOutcomes, which will allow contractors to track their progress with achieving the quality payments criteria. The assessment also contains links to the PSNC website to provide contractors with additional information on each criterion and links to other PharmOutcomes services to assist contractors with collating evidence.

Once a contractor has logged into PharmOutcomes, the framework can be accessed by clicking on the ‘Assessments’ tab on the PharmOutcomes homepage.

Quality payment criterion: asthma referrals

An asthma referral tool has also been created to assist contractors in meeting the quality payment criterion, ‘On the day of the review, the pharmacy can show evidence of asthma patients, for whom more than 6 short acting bronchodilator inhalers were dispensed without any corticosteroid inhaler within a 6 month period, are referred to an appropriate health care professional for an asthma review’.

This PharmOutcomes tool allows contractors to record patient details who have consented to be referred to their GP practice because of them having been dispensed more than 6 short acting bronchodilator inhalers without any corticosteroid inhaler within a 6 month period. When this data is saved on PharmOutcomes a referral will automatically be sent to the patient’s GP practice (if a secure email address is held for that GP practice on PharmOutcomes).

Once a contractor has logged into PharmOutcomes, the tool can be accessed by clicking on the ‘Services’ tab on the PharmOutcomes homepage.

Healthy Living Pharmacy Level 1 framework

PharmOutcomes log in details

If a contractor has not used PharmOutcomes before, log in details can be obtained by sending a message to the PharmOutcomes helpdesk. Contractors will need to provide details of the pharmacy, contact information and an email address to issue the login details to.

Health Living Pharmacy Level 1 support

If an LPC or a commissioner has a full PharmOutcomes licence, they can make available a HLP Level 1 framework for contractors to access on PharmOutcomes free of charge. This framework will allow contractors to track their progress with achieving the HLP quality criteria. If contractors would like to consider using the framework, they should contact their LPC to see if this is available in their area.

Where the assessment has been made available by a commissioner, contractors who own multiple pharmacies can view the progress that each of their pharmacies are making to achieving the HLP Level 1 quality criteria by using their PharmOutcomes head office/administrator viewer login.

Where an LPC has a licence for PharmOutcomes, they will be able to view the progress their contractors are making to achieving the HLP Level 1 criteria by making the framework available. This will help identify additional support that contractors require.

To support the HLP initiative, Pinnacle Health LLP has introduced a new low cost HLP licence starting at £995 plus VAT. This allows organisations such as LPCs to make the assessment available across a whole area. The licence type provides access to the communications functionality and the assessment functionality only. Please contact the PharmOutcomes helpdesk for more information.

If a contractor requires PharmOutcomes login details for one of their team members, these can be obtained by sending a message to the PharmOutcomes helpdesk. Contractors will need to provide details of the pharmacy, contact information and an email address to issue the login details to.

If a contractor does not have a head office/administrator viewer login this can be obtained by sending a message to the PharmOutcomes helpdesk. Contractors will need to provide details of pharmacy sites, contact information and an email address to issue the login details to.

Transfer of care options

PharmOutcomes can now offer three options to support transfer of care from hospital to community pharmacy ranging from the original web based solution, partial integration with hospital systems, to a fully integrated approach.

Pinnacle Health LLP has sent their brochure detailing the three options to all hospital trusts and Clinical Commissioning Groups in England, to alert them of this development. They have also sent a ‘cheque’ which highlights that just 10 referrals a day sent from hospital teams to community pharmacies for follow up review, after patient discharge can save NHS organisations hundreds of thousands of pounds a year. Just 10 referrals a day, 5 days a week, produces  over a million pounds of efficiency savings annually for the local health economy.

PharmOutcomes MasterClasses

Pinnacle Health LLP hold local interactive PharmOutcomes MasterClasses at regular intervals which are ideal opportunities for commissioners, LPCs and other local representatives to learn what the system is capable of and how maximum benefit can be derived.

The MasterClasses provide a complete system overview, a full explanation of all new functionality including real time reports, finance, data extraction and an opportunity to use the system and build a service.

There are two different types of MasterClasses:

Beginners: This session covers how to create services, manage users, an end system walk through covering communication, service design, reports and claims.

Advanced: Attendees must have attended the beginners’ class to get the most out of this training day. This class covers GP notifications, referrals, enrolments, SMS reminders and conditional questions.
