Please note this service is taking across two London LPCs. Therefore, the following LPCs have the same CIS in their areas:

  • Ealing, Hammersmith & Hounslow LPC (Hammersmith & Fulham area); and
  • Kensington, Chelsea & Westminster LPC.

The Community Independence Service (CIS) is a provider partnership which focuses on hospital admission avoidance where clinically appropriate and care can be provided in the community. CIS facilitates early supported discharge from hospital, maximises independence and reduces dependency on longer term services.

As rapid access to medicines is often required, the CIS GPs or NMPs (non-medical prescribers) will prescribe on FP10s and then email then to the community pharmacy (via nhs mail) in advance of sending the hard copy of the FP10 (either by hand when the medicines are collected, or by registered mail within 48 hours).
