Description
This community pharmacy locally commissioned service is provided to support the London Borough of Hillingdon reablement service and home care service providers from whom the task of ‘administration of medication’ Level 2 intervention is required.
Pharmacy contractors will provide Medication Administration Record (MAR) charts each time a new prescription is received.
Upon dispensing of the prescription, the MAR is produced by applying an additional set of dispensing labels to the MAR which correspond to the medicines dispensed. The times for administration of the medication are indicated by ticking in the ‘Time’ box corresponding to AM, Lunch, Tea or Night.
The MAR is supplied to the reablement team/service provider representative or family member/informal carer with the dispensed medication along with any additional information, written and verbal, that would be provided in accordance with national Essential Service specifications and terms of service.
