Description
‘HELP FOR HARRY’ is facilitating safe transfer of care by supporting patients to manage and take their medication safely when discharged from hospital. It refers a patient for on-going support from their regular community pharmacist. This support may be provided through one of the Advanced Services they offer - Medicines Use Review (MUR) or New Medicine Service (NMS). This service gives the patient help and advice which is tailored just for their needs so they can maintain a positive health outcome.
