East & North Herts CCG and Herts Valleys CCG are undertaking pilot projects to explore the provision of  a  new service  Community Pharmacy Contractors. The service is designed to improve patient safety by supporting the production and checking of a MAR ((Medicines Administration Record) chart for specific patients receiving support at home in their medicines administration. The pharmacist will be responsible for the production & maintenance of MAR for a specific cohort of patients identified by the local authority whilst ensuring a high level of patient care and provision of information for audit and evaluation.

The pilot is limited to an initial cap of 25 patients per community pharmacy. This may be increased during the course of the pilot depending upon activity.

This pilot was completed 1st August 2015.
