Description
This service is aimed at increasing the detection of patients with atrial fibrillation (AF) and hypertension in order to facilitate referral and management by their GP practice. The pharmacy contractor will screen eligible patients for AF and hypertension and make appropriate referrals to GP practices.
This service is designed to be delivered alongside the NHS Health Check national screening programme which may result in referral to or from the locally commissioned NHS Health Check programme.
