Description
The aim for the Medicine Support Service is for hospital and community pharmacy teams to implement new electronic patient referral systems.
Previously known as the ‘Hospital to Home’ Service, this is now being project managed by the South West Academic Health Science Network and to align with similar services across the South West re-launched as ‘The Medicines Support Service’.
After hospital discharge, a consenting patient's discharge summary will be sent via PharmOutcomes to a community pharmacy of choice with a referral for the Medicines Use Review service or the New Medicine Service.
