The aims of this service are to improve health outcomes for patients who experience difficulty in getting to a community pharmacy for the purposes of a MUR (ie. housebound or usually resident in a care home). The service also aims to demonstrate the role that community pharmacy teams can play in raising quality and improving efficiency. This will be achieved by:

  • maximising the MUR allocations available per community pharmacy contractor and waiving the requirement to submit PREM notification forms for each and every patient where a domiciliary or telephone MUR is required and in doing so, improve access to MURs for patients who experience difficulty in getting to their community pharmacy;
  • increasing patient and carer understanding of their medicines, identifying risks of polypharmacy and adherence issues and working to resolve these to facilitate improved health outcomes; utilising the evidence-based Polypharmacy Stopp/Start approach;
  • supporting carers by signposting them to appropriate sources of help and advice;
  • identifying and addressing factors contributing to waste, such as stock-piling and working with carers, prescribers and the community pharmacy team to reduce unnecessary supply;
  • facilitating better use of skill-mix across the community pharmacy team by encouraging pharmacy technicians to have a role in the service in regards to medicines reconciliation; and
  • fostering positive working relationships between GP practices and community pharmacy teams.
