Tier 1 Service
The aim of the service is to support anticipatory prescribing and allow rapid access to medicines commonly prescribed in palliative care, to enable a greater percentage of patients to have a home death, rather than to be moved to an institution such as hospital. By establishing a network of pharmacies that will routinely hold stock of the four main End of Life medicines, carers and other health care professionals should not experience delays in obtaining these medicines for the patients that they are caring for.

Tier 2 Service
The choice to prescribe and administer tier 2 medicines to palliative care patients requires specialist support from the DMH/ UHNS/ SSOTP/ specialist GP and it is anticipated the tier 2 service will help support the teams to reliable and timely access to these medicines in the primary care setting.

No formal accreditation is required although pharmacists are responsible for their continuing professional development and therefore the CPPE pack (Palliative Care 2nd Edition Ref 42007) is recommended.
