Services Database Pharmacist Intervention Scheme (Herefordshire Area)
The aim of the service is to increase efficiencies within prescribing and dispensing of NHS prescriptions to reduce the number of unwanted medicines dispensed and therefore wasted. As part of the service, if a medicine is not required, it will not be dispensed.
The pharmacist will discuss the prescribed medicines with the patient, or their authorised representative to identify any item that is not required. In addition the intervention scheme will bring to the attention of prescribing support pharmacists/prescribers points to amend on prescriptions to increase cost effectiveness and reduce risks associated with medicines.
