Pharmacy Care Plan service

  • Each eligible patient will have a one-to-one discussion with a pharmacist. Patients will explore and identify the health goals that are important to them
  • The pharmacist will help the patient draw up a Patient Care Plan setting out individual goals and steps to be taken to achieve them. The pharmacist will supply the patient with relevant information guides, prepared by leading patient support groups
  • Baseline data on general health and wellbeing as well as measurements of specific health conditions will be collected
  • Information will be recorded in the Patient Care Plan. The patient will own this document and bring it with them to subsequent consultations
  • The pharmacist will assess the patient’s adherence to their prescribed medicines and their skill with any devices (inhalers) being used. Appropriate support, advice and information will be given
  • Medicines optimisation will be discussed using STOPP/START and other validated clinical tools for falls and pain management, as appropriate. Recommendations will be made to prescribers for changes in medication, where clinically indicated
  • Patients will receive ongoing support from the pharmacy team whenever they return to collect prescribed medication. Progress towards health goals will be measured and new goals agreed where appropriate
  • Patients can also visit the pharmacy at any time to discuss ongoing health and medicines-related issues
