Description

Please note this service is commissioned across Derbyshire and Nottinghamshire by NHS England North Midlands. Therefore, the following LPCs have the same domiciliary Medicines Use Review service in their areas:

  • Derbyshire; and
  • Nottinghamshire.

The service has only been added onto the Services Database under this entry to avoid duplication.

This service will allow the provision of Medicine Use Reviews in the home setting rather than in the pharmacy for patients with long term conditions who are housebound and those unable to attend the pharmacy for a review, who are taking six medicines or home.

The service is scheduled to run from 1st April 2017 to 30th September 2017.

Patients can self-refer, be identified as suitable by GPs, pharmacists and their staff, by carers or by secondary care professionals.
