Description
Pharmacists operating under this service level agreement will provide this service only to patients registered on the “Gluten free supply” module of the Servicepact system. The pharmacy will supply any gluten free product included in the formulary to the patient in line with the limitations (as stated in appendix 1 of the service specification). Sufficient quantities of gluten free food should be supplied on one occasion so that the same item will not need to be supplied again within 28 days.
All supplies must be recorded on the Servicepact system. The pharmacy must collect any charges due for the supply of gluten free products as if they were supplied on prescription. The value of such charges must be entered onto the Servicepact system.
