Services Database Transfer of Care Service (Includes 2 LPCs)

Please note this service is available across Buckinghamshire and the Thames Valley. Therefore, the following LPCs have the same transfer of care service in their areas:

• Buckinghamshire LPC; and
• Thames Valley LPC.

The service has only been added onto the Services Database under this entry to avoid duplication.

Briefly the service involves:

• the hospital pharmacy identifies a patient and completes a referral form on PharmOutcomes;
• the patient identifies the pharmacy they wish to use;
• an email is sent to the pharmacy telling them to check PharmOutcomes;
• the pharmacy assesses the referral and accepts or rejects it;
• the pharmacy contacts the patient;
• the pharmacy carries out a review, very often an MUR and claims the fee for that in the usual way; and
• the pharmacy completes a form on PharmOutcomes.

An audit will be carried out to assess the impact of the service on areas such as hospital admission.
