Description

The Transfer of Care service is a new service which aims to ensure patients receive appropriate support from their community pharmacist soon after leaving hospital.

Community pharmacy and hospital pharmacy colleagues in the North East have been working together to try and improve care for recently discharged patients, using PharmOutcomes to pass messages to and from the hospital and community pharmacy.

A patient nearing discharge who would potentially benefit from a further intervention delivered by their community pharmacy is identified.

The interventions the hospital can select are:

  • Medicines Use Review;
  • New Medicine Service;
  • repeat dispensing service;
  • home delivery service;
  • Appliance Use Review;
  • stop smoking service; and
  • flu vaccination (September - March).
