Transfers of Care Around Medicines (TCAM) Service

Transfer of Care Around Medicines service ensures support around medicines for patients being discharged from a hospital setting, giving them the extra support they need when taking current and newly introduced medication.

Extra support may be needed because patients’ medicines have changed or they just need additional help to make sure they are taking their medicines safely and correctly. Statistics show around 60% of patients have three or more changes made to their medicines during a hospital stay and only 10% of elderly patients will be discharged on the same medication they were on previously.

Feedback has highlighted from patient’s that they don’t always remember everything they are told in hospital, so it can be very helpful to have someone go through it again, discussing side effects and checking they understand.

The transfer of care process is associated with an increased risk of adverse effects. 30 -70% of patients experience unintentional changes to their treatment or an error is made because of a miscommunication.

This is what the Transfers of Care Around Medicine (TCAM) project aims to address.

When patients in hospital are identified as needing extra support with their medicines, they are referred through a safe and secure digital platform when they’re discharged for advice from their local community pharmacist.

Original work in the North East has shown that patients who see their community pharmacist after they’ve been in hospital are less likely to be readmitted and, if they are, will experience a shorter stay.

Locally, through Imperial College Health Partners (ICHP) & KCW & Pan-London LPCs the following NHS trusts will be involved in having transfer of care around medicines pathway;

How to access PharmOutcomes 

Obtain your PharmOutcomes Activation Code here: No code provided, it appears there is already and account for this store.

Many of you may have already accessed PharmOutcomes®, but some have not. If you have not yet accessed the system and do not have a login please follow the actions below;

If you are already using the PharmOutcomes® system then you can use your existing login details straight away to log in and go to the Services menu once logged on to PharmOutcomes®. There are some useful service guides available from the “Help” tab to talk you through login and data entry, etc.

If you do not remember your PharmOutcomes login details, or you are new to the system then please use the information below to gain immediate access. If you have any problems accessing the system or have questions about how to use it, please contact the dedicated support service via the Help tab at

Once logged in go to your services tab. You will see the services you are accredited to provide in the left-hand side bar. When you have opened the service question set, you will find a link to the service guide in the left hand side bar. Please read this carefully prior to commencement of the TCAM service delivery.

How to get a new username and password if you haven’t used the new PharmOutcomes system

1.    Using a web browser, go to and click on Help in the grey menu bar.

2.    If you have been sent an activation code, from the help tab at the bottom left of the screen, click the link under the orange heading “Activation code”, this reveals a box entitled Activate Account. Enter your activation code and click the orange button to activate account. 

3.    The system will then show you which pharmacy this code is associated with and you should complete the details on the screen as below, then press Send Password.

Make sure you can access emails sent to the Email Address you enter on the screen as your password will be sent to this email address immediately.

4.    You can then log into the system with your username and password that will be sent to you by email.

5.    When you first log in, you will be asked to reset your password and select a security word – do not forget this word as there is no means to automatically reset it.
