Patient safety is the avoidance of unintended or unexpected harm to people during the provision of health care. We support providers to minimise patient safety incidents and drive improvements in safety and quality. Patients should be treated in a safe environment and protected from avoidable harm.

There is a new website that provides a forum for community pharmacy organisations to openly share and learn from each other when things go wrong and considers how learning from patient safety incidents can be applied across the pharmacy network and wider NHS.  It can be found at the following link
The Community Pharmacy Patient Safety Group provides a forum for community pharmacy organisations, competitors in a commercial sense, to openly share and learn from each other when things go wrong, as well as from other sectors and industries.They are a self-funded, self-created Group with a difference.  They consider how learning from patient safety incidents can be applied across the pharmacy network and wider NHS, and then create the opportunities and resources to do just that.A group of MSO’s (medicines safety officers) have been assigned as part of the group representing larger organisations.The MSO for independent community pharmacists is Leyla Hannbeck from the NPA

The NPA have produced a hub page with access to quarterly newsletters and useful toolkits to manage patient safety incidents and also toolkits to help support management of major patient safety identified themes in your pharmacies

Responsibilities for supporting pharmacy businesses and superintendents include:

  • Promoting the safe use of medicines across the pharmacies
  • Implementing local and national medication safety initiatives
  • Improving patient safety on a day-to-day basis
  • Submitting medication error reports
  • Improving reporting and learning from medication incidents
  • Responding to requests from the Patient Safety Domain in NHS England and Medicines Healthcare product Regulatory Agency (MHRA) for further information about medication error incidents.
