Medicine Supply Notification: Dalteparin (Fragmin® ) 10,000units/4ml solution for injection ampoule

Department of Health and Social Care (DHSC) has issued a medicine supply notification for Dalteparin (Fragmin®) 10,000units/4mL solution for injection ampoule.

Date of issue 21st February 2023

  • Dalteparin (Fragmin®) 10,000units/4mL solution for injection ampoule is due to be out of stock from mid-February 2023 until late March 2023 whilst the ampoules are converted into a vial presentation.
  • Patient safety precautions including the review of local protocols and appropriate education and training for healthcare professionals, need to be implemented for the duration of this shortage. This includes ensuring the higher concentration multidose vials of dalteparin (Fragmin®) 100,000units/4mL (10 times the strength) are not used as an alternative option to the product in shortage to reduce the risk of dosing errors.
  • Regional Pharmacy Procurement Specialist (RPPS) may be able to facilitate mutual aid between hospitals.
  • Please note there is also discontinuation of dalteparin (Fragmin ®) 10,000units/1mL solution for injection ampoule in May 2023 (MSN/2022/109)

A copy of this medicine supply notification, including further information for pharmacy teams, has been sent to all pharmacy NHS email addresses.

DHSC and NHSE/I have now launched an online Medicines Supply Tool, which provides up to date information about medicine supply issues. The contents of these MSNs can now be viewed on the Tool.

The Tool also details any changes to resupply dates and updates to the entries.
