Dementia Together

Dementia Together is service which launched in 2017 to help people living with dementia and their family carers access the support and information they need to live well for longer.  The service is open to anyone registered with a GP surgery in Suffolk, excluding Waveney.

The service is designed to be a single point of contact for people at any stage of the condition.  Anyone can contact the Helpline who may just be curious and have questions about dementia to those that may be feeling concerned and in need of more practical support.  Dementia Navigators are also able to provide home visits.

Sue Ryder is delivering the service in partnership with local Dementia Action Alliances and health and social care organisations, to ensure people receive joined up support.
