Discharge Medicines Service (DMS)

The Discharge Medicines Service (DMS) becomes a new Essential service within the Community Pharmacy Contractual Framework (CPCF) from 15th February 2021.

The service seeks to ensure better communication of changes made to a patient’s medicines in hospital and its aims are to:

  • Optimise the use of medicines, whilst facilitating shared decision making;
  • Reduce harm from medicines at transfers of care;
  • Improve patients’ understanding of their medicines and how to take them following discharge from hospital;
  • Reduce hospital readmissions; and
  • Support the development of effective team-working across hospital, community and primary care networks pharmacy teams and general practice teams and provide clarity about respective roles.


Addenbrooks Hospital

Addenbrookes Hospital will go live from 5 July with the Discharge Medicines Service. An engagement for community pharmacy teams has been arranged for 23 June from 7.30pm – 8.30pm.

This will be a web based referral with an attached discharge summary which will be sent via PharmOutcomes.
We will aim to share a recording of the webinar and any further updates as we receive them.

DMS Presentation
Virtual meeting 1-2pm or 8-9pm on 30th June 2021Norfolk LPC is supporting two virtual meetings to talk through the Discharge Medicines Service and these events are also open to Suffolk pharmacies.

They are being presented by Richard Brown where he will be discussing the Discharge Medicines Service process, including an AF anticoagulation Case Study. This will be practical training based on looking at what makes a good DMS interaction.

There are two available time slots, 1-2pm or 8-9pm, consisting of a 30-45 min presentation followed by at least 15 Q&A time

All pharmacies will receive information via their NHS premises accounts about these events.
